Scaffold Inspection Certificate???

Anyone help with a basic scaffold inspection form/certificate??

Jobs been handed over, safety officer is asking for a register for recording regular inspection. Normally state it's the clients responsibility once handed over but were going to do it this time

don't fancy an evening on Publisher!

Any help much appreciated
