hello lads
a job I am starting soon has asked for a dismantling methodology (in line with sg4?) and also a rescue plan (in the unlikely event that a scaffolder ever be suspended) and as I am quite new to this paper work side of the job I would be grateful for any advice
 
A rescue plan is needed on all jobs, methodology will all be dependent on type of job you are doing. It's all to do with syncope.

If it's a straight forward job, it's just self rescue on to a lower lift. A method statement is simple enough as well. Just tell them that your erecting it to tg20 and using a step or whatever to comply with sg4.
 
I have different ones, all depends on if its site work, street or industrial as you will need a rescue kit as well. Speak to safety and access they can provide you with methods.
 
Build additional Scaffold


In the event of a person falling from an independent, tower, birdcage scaffold, or any other structure where there is access adjacent to the suspended person then the guidance below is to be followed.


Rescue Scenario 1:

Person falls from the work platform and is suspended by their harness & lanyard but is still conscious.

Site / other emergency services to be informed as soon as practicable. The conscious suspended person will be able to assist in their own rescue, the other team members will move to the working platform adjacent to the suspended person clip to the inside handrail or upper / lower ledger or other suitable anchorage point and assist the suspended person in getting onto the working platform, if the working platform is a non-working scaffold lift then they will lay a safe working platform a minimum of 4 boards wide (depending on the width of the scaffold) they will be clipped on at all times & as the structure has been built to the guidance laid out in SG4 there will be an inside and outside handrail these must remain in place at all times. Once the person is inside the handrails & on a safe platform then the suspended person’s karabiner can be unclipped or the lanyard cut from the anchor point to which it is attached. Emergency services should then make their way to the casualty and make an assessment as to the condition of the casualty and decide on the next steps to get the casualty to ground level, either using the site emergency response team, / Gotcha rescue kit or the external rescue services i.e. fire Brigade.


Rescue Scenario 2:

Person falls from the work platform and is suspended by their harness & lanyard but is unconscious or cannot assist in their rescue due to injury.

The other scaffolder(s) in the gang immediately contact Site / other emergency services as soon as practicable. The scaffolders will move to the lift adjacent to the suspended person clip to the inside handrail or upper / lower ledger and recover the suspended person onto the working lift, if the lift is non-working then they will create a safe working platform of a minimum of 4 boards wide (depending on the width of the scaffold) they will be clipped on at all times & as the structure has been built to the guidance laid out in SG4 there will be an inside and outside handrail these must remain in place at all times. Once the person is inside the handrails on a safe platform then the scaffolder’s karabiner can be unclipped or the lanyard cut from the anchor point to which it is attached and steps taken to ensure their airway is open. Emergency Services Medical Representative should then make their way to the working platform to make an assessment of the condition of the injured person. The decision will then be taken how to get the injured person down to grade either using the site / emergency response team, / Gotcha rescue kit or the external rescue services i.e. fire Brigade.

Rescue Scenario 3:

Person collapses on the work platform.

Site / other Emergency Services to be informed as soon as practicable that there has been an incident. Emergency services should then make their way to the casualty and make an assessment as to the condition of the casualty and decide on the next steps to get the casualty to ground level, either using the site emergency response team, / Gotcha rescue kit or the external rescue services i.e. fire Brigade.

Rescue of a person using a Retractable Inertia Block.


In the event of a person falling from a cantilevered / suspended scaffold, or any other structure where there is no access adjacent to the suspended person then the guidance below is to be followed.


Rescue Scenario 1:

Person falls from the work platform and is suspended by their harness attached to the inertia reel
but is still conscious.

Site / other Emergency Services to be informed as soon as practicable. A suspension loop is to be held at all times on the working platform, the suspension loop will be passed to the suspended person to ensure circulation is maintained whilst suspended.

At least 2 of the team members will be trained in the use of the recovery inertia reel. The trained team members will themselves remain clipped on whilst carrying out the rescue procedure until they are in a safe hand railed zone. The trained team members will raise the individual back onto the working platform using the mechanical arm which is attached to the recovery inertia reel.

The rescuers will then take the suspended persons weight and move them to a safe hand railed zone away from the leading edge the suspended person can now be unclipped. If the individual has sustained any injuries these must be assessed before they are allowed to egress the structure. In the event that their injuries prevent them from been able to egress the structure safely, a decision will be reached by the rescue team as to how to get the injured person to ground level, either using the site emergency response team, or the external rescue services i.e. fire Brigade.

Rescue Scenario 2:

Person falls from the work platform and is suspended by their harness attached to the inertia reel but is unconscious or cannot assist in their rescue due to injury.

Site / other Emergency Services to be informed as soon as practicable. At least 2 of the team members will be trained in the use of the recovery inertia reel.

Using “Gotcha” Rescue Kit


In the event of a person falling from a cantilevered / suspended scaffold, or any other structure where there is no access adjacent to the suspended person then the guidance below is to be followed.


Rescue Scenario 1:

Person falls from the work platform and is suspended by their harness & lanyard but is still conscious.

Site / other Emergency Services to be informed as soon as practicable. A Spanset ‘Gotcha’ system is held at all times on the working platform, along with suspension loop which is passed to the suspended person to ensure circulation is maintained whilst suspended. At least 2 of the team members are trained in ‘Gotcha’ Rescue, one of whom immediately sets up the system. When the system is attached as per the manufacturer’s instructions to a suitable anchorage point & to the suspended persons harness, the rescuers then take the suspended persons weight, the suspended person’s karabiner can be unclipped or the lanyard cut from the anchor point to which it is attached. Providing the suspended person is still conscious and their airway is not impaired they will then be lowered to the ground (If it is safe to do so and the area below is clear from obstructions) where the Emergency Services will be waiting. If for any reason they cannot be lowered to grade, they must be pulled up to the nearest suitable working platform, and their injuries, if any, assessed before they are allowed to egress the structure. In the event that they are injured and cannot egress the structure, a decision will be reached by the rescue team as to how to get the injured person to ground level, either using the site emergency response team, / Gotcha rescue kit or the external rescue services i.e. fire Brigade.

Person falls from the work platform and is suspended by their harness & lanyard but is unconscious or cannot assist in their rescue due to injury.

Emergency services should make their way to the casualty and make an assessment as to the condition of the casualty and decide on the next steps to get the casualty to ground level, either using the site emergency response team, / Gotcha rescue kit or the external rescue services i.e. fire Brigade.

The use of a MEWP to assist in the rescue of a person who has fallen from height

Site / other Emergency Services to be informed as soon as practicable, this method of rescue can be used with either a conscious or unconscious person. The MEWP (i.e. scissor lift or cherry picker) should be located close to the activity and a suitably qualified operator will be in the vicinity (this person may be part of this or another working team).

Once in position and the ground conditions assessed taking into consideration the height restrictions and obstacles the MEWP will locate itself under the suspended person and elevate upwards taking the casualties weight, thus allowing the lanyard to be removed from its anchor point.

The MEWP should then without hesitation be lowered to ground level, if unconscious the casualty should be assessed and steps taken to ensure the airway is open in all cases await Emergency services who will make an assessment as to the condition of the casualty and decide on the next steps.


The rescue of a person who is working from the confines of a MEWP and unable to operate machine themselves

Site / other Emergency Services to be informed as soon as practicable a suitably qualified operator will be in the vicinity (this person may be part of this or another working team). The operator will take control of the MEWP (i.e. scissor lift or cherry picker) utilising the base controls of the MEWP.

The MEWP should then without hesitation be lowered to ground level, if unconscious the casualty should be assessed and steps taken to ensure the airway is open in all cases await Emergency services who will make an assessment as to the condition of the casualty and decide on the next steps.

Erect/dismantle procedure below is a bit dated and you will need to change some of it to make it current to the regs and guidance.

1.General Access Scaffolds to TG20 -08
2, SCAFFOLD FOUNDATIONS THE CLIENT SHALL PROVIDE ADEQUATE LEVEL, CLEAR OF DEBRIS FOR THE TYPE OF SCAFFOLD REQUIRED. CLIENT SHALL CLEARLY INDICATE THE LOCATIONS OF ALL EXISTING / PROPOSED SERVICES PRIOR TO THE ERECTION OF THE SCAFFOLDING. TIMBER SOLEPLATES TO BE IN ACCORDANCE WITH CLAUSE 7.5 – TG20 : 08 FOR THE TYPE OF EXISTING GROUND CONDITIONS ENCOUNTERED. OR FOUNDATIONS AS SPECIALLY DESIGNED BY OTHERS.
3. ALL SCAFFOLD TO BE ERECTED AND DISMANTLE IN ACCORDANCE WITH TG20 -08, WIND LOADING TO BE IN ACCORDANCE WITH BS 6399 PART 2.
4. ALL SCAFFOLD EQUIPMENT SHALL BE IN GOOD WORKING ORDER AND CONFORM TO RELEVANT DSL STANDARD OR CURRENT BRITISH STANDARD.
5. SCAFFOLDING ERECTION AND DISMANTLING SHALL BE CARRIED OUT USING SAFE WORKING PRACTICES, WITHIN THE SCAFFOLD STRUCTURE USING A MIN OF 4 BOARDS WIDE WORKING PLATFORM & A SINGLE GUARDRAIL ON A 5-BOARDS WIDE SCAFFOLD. ALL SCAFFOLDERS SHALL CONFORM TO DSL HARNESS CODE OF PRACTICE & GUIDANCE LAID DOWN BY THE N.A.S.C IN THE CODE OF PRACTICE SG4.
6. SCAFF-STEP
USING THE SCAFF-STEP SYSTEM WILL ENSURE OUR OPERATIVES ARE WORKING BEHIND A SINGLE GUARD RAIL ON A FULLY BOARDED PLATFORM AT ALL TIMES REDUCING THE NEED FOR CLIPPING ON AND OFF (TUNNELLING / TRAVERSING) AND ALSO ENSURING COLLECTIVE PROTECTION AT ALL TIMES. EACH SCAFFOLDER WILL BE FULLY TRAINED IN THE SAFE USE OF THE SCAFF-STEP
THE SCAFFOLD FRAMEWORK FOR THE NEXT LIFT UPWARDS WILL BE INSTALLED IE TRANSOMS & LEDGERS WHILST WORKING OFF THE EXISTING FULLY BOARDED PLATFORM. THE STEP IS USED TO ERECT A SINGLE GUARD RAIL ON THE LIFT ABOVE IN ADVANCE OF THE OPERATIVE ACCESSING THE FRAMEWORK. IT IS USED BY PLACING THE FEET OF THE SCAFF-STEP ON THE FULLY BOARDED PLATFORM OR FLOOR IF AT GRADE AND SECURING THE TOP SECTION AGAINST THE TOP GUARD RAIL OF THE LIFT, THIS ALLOWS THE SCAFFOLDER TO CLIMB THE STEP WHILST CLIPPED ON TO THE REAR UPPER LEDGER (THIS IS DONE BEFORE CLIMBING THE SCAFF-STEP), & PASS THROUGH THE FRAMEWORK OF THE LIFT ENABLING THE SCAFFOLDER TO ERECT A SINGLE GUARD RAIL WHILST THE LEDGER ABOVE BECOMES THE GUARDRAIL FOR THIS PROCESS, ONCE THE SCAFFOLDER IS STOOD ON THE TOP STEP OF THE SCAFF STEP THEY WILL ATTACH THE FRONT KARABINER TO THE FRONT LEDGER, THIS WILL ACT AS A RESTRAINT SHOULD THEY LOOSE THEIR BALANCE. ONCE COMPLETE THE BOARDS FROM THE BOTTOM LIFT ARE MOVED AND PLACED ONTO THE TRANSOMS ABOVE (THE SCAFFOLDER MUST BE CONNECTED BY THEIR LANYARD TO THE UPPER LEDGER AT ALL TIMES WHILST MOVING THE BOARDS, THE SCAFFOLDER WILL THEN ACCESS THE NEXT LIFT BY MEANS OF EITHER UTILISING THE INSTALLED LADDER, LADDER TOWER OR STAIRCASE) THIS WILL PERMIT THE SCAFFOLDER WHEN ACCESSING THE LIFT AND BE FULLY PROTECTED BEHIND A SINGLE GUARD RAIL. THIS PROCESS IS REPEATED LIFT TO LIFT
7. ALL SCAFFOLD MATERIALS WILL BE TRANSPORTED TO OR AS NEAR TO WORK AREA AS POSSIBLE BY TRACTOR AND TRAILER / FLT OR OTHER SUITABLE MEANS. MATERIALS WILL BE STACKED IN A NEAT AND TIDY MANNER USING STILLAGES AND BINS WHEREVER POSSIBLE. MANUAL HANDLING GOOD PRACTICE SHALL BE OBSERVED.
8. ALL DOORS, EXITS, STAIRCASES, WALKWAYS, FIRE AND EMERGENCY EQUIPMENT POINTS SHALL REMAIN UN-OBSTRUCTED.
9. ALL SCAFFOLDS ERECTED TO THE TOLERANCES CONTAINED IN TABLE 9 – TG20 -8
10. LEDGERS TO WORKING LIFTS TO BE SECURED TO STANDARDS USE ONLY RIGHT ANGLED LOAD BEARING COUPLERS TO CLASS A OR B. ALL JOINTS TO BE STAGGERED USE ONLY SLEEVE COUPLERS FOR THE CONNECTIONS. UNLESS OTHERWISE NOTED.
11.TRANSOMS SPACING AND FIXINGS TO COMPLY WITH TABLE 8 TG20 -08.
12. SCAFFOLDING ERECTED TABLE 9 TG20 -08 OR AS OTHER WISED STATED ON DESIGN DRAWING(S)
13. LADDERS WILL BE STEEL OR WOODEN POLE SUPER LADDERS WIRED EVERY RUNG, PITCHED ON A FIRM LEVEL BASE , FIXED AT AN ANGLE OF 4 TO 1 WHEREVER PRACTICABLE AND WILL BE INSTALLED INTERNALLY TO THE SCAFFOLD WHEREVER PRACTICABLE. LADDERS SHALL BE SECURELY LASHED OR FIXED, TO PREVENT MOVEMENT. LADDERS TO BE INSTALLED AS SCAFFOLD IS ERECTED. LADDER ACCESS POINTS TO PLATFORMS WILL BE BUILT IN ACCORDANCE WITH PREFERRED SITE PRACTICE.


14. WORKING PLATFORM TO BE CLOSE BOARDED FITTED WITH TOEBOARDS AND STOP END BOARDS AND GUARDRAILS TO CORRECT HEIGHTS. WHERE THE PLATFORM IS AWAY FROM THE WORK SURFACE INTERNAL TOEBOARDS AND GUARDRAILS SHALL BE INSTALLED, UNLESS OTHERWISE INSTRUCTED BY THE CLIENT (whereby they would need to ensure that a safe system of work is employed & adhered to). ALL PLATFORMS MUST BE BIG ENOUGH FOR WORK TO BE CARRIED OUT SAFELY. BOARDS IN PLATFORMS WILL BE SECURED AGAINST DISPLACEMENT.
B. DISMANTLE PROCEDURE.
1. ENSURE THAT ALL TIES AND BRACINGS ARE IN PLACE AND ALL TRADE DEBRIS HAS BEEN REMOVED FROM WORKING PLATFORM. DISMANTLING WILL NOT TAKE PLACE UNTIL TRADE DEBRIS IS REMOVED BY OTHERS. DO NOT TURN BOARDS TO REMOVE TRADE DEBRIS!
2. DISMANTLING SHALL BE SYSTEMATIC AND PROGRESSIVE.
3. PLATFORMS SHALL NOT BE OVERLOADED WITH DISMANTLED MATERIALS.
4. MATERIALS SHALL BE LOWERED TO GRADE NO MATERIALS WILL BE DROPPED OR THROWN TO GROUND LEVEL.
5. ALL MATERIALS SHALL BE STACKED SAFELY PRIOR TO REMOVAL. ALL MATERIALS WILL BE REMOVED PROMPTLY.
6. LADDERS WILL BE LEFT INSITU FOR AS LONG AS POSSIBLE TO AID DISMANTLING
