A scaffold collapse in Brooklyn on Tuesday sent a worker plummeting five stories to his death - and left two others dangling from a rope for several minutes, city officials said.

Henyrk Siebor, 42, of Brooklyn and two other brickworkers were working on the facade of an apartment building at 438 12th St. in Park Slope when the scaffold broke about 5:35p.m., police said.

Siebor plunged to his death when the line snapped, LiMandri said.

The other two workers, who were not identified, clung to the scaffold for dear life until firefighters from Ladder 122 pulled them to safety through a fourth-story window.

"Two people are very lucky to be alive," said city Buildings Department Commissioner Robert LiMandri.

Speaking in Polish through sobs of grief, a cousin of Siebor's described him as a hard worker who sent his pay home to his wife and three children in his native Poland.

"She is still in shock," said Hanna Dawidowicz, 48, a neighbor who translated for the cousin, who lives in Greenpoint, Brooklyn, and did not want to give her name.

Siebor's son Robert, 16, was staying with his father for the summer and was at the scene within minutes of the accident.

LiMandri said the cause of the accident was under investigation.

The facade work was being done by Nova Restoration of Brooklyn, LiMandri said.

The workers had set up two scaffolds side by side, LiMandri said. The workers were all on the same scaffold as it gave way when Siebor attempted to cross to the other one, LiMandri and police said.

The building has 10 outstanding Buildings Department violations, including five for work done without a permit.
